Description
What Is Ristova Injection?
Ristova injection is a prescription medicine produced by Roche Pakistan Limited. Its active ingredient, rituximab, is a chimeric monoclonal antibody that targets the CD20 antigen on B-lymphocytes. When rituximab binds to CD20-positive B-cells, it triggers their destruction through your immune system’s natural defense mechanisms.
You receive Ristova inj in two strengths:
- 100mg (2 vials x 10ml)
- 500mg (1 vial x 50ml)
Both are administered only by intravenous (IV) infusion under the supervision of qualified healthcare professionals.
Ristova Injection Uses
Doctors prescribe Ristova injection for several serious hematological and autoimmune conditions. You should always consult your doctor to confirm if this medicine is right for your diagnosis.
Blood Cancers
- Non-Hodgkin’s Lymphoma (NHL): Ristova injection treats CD20-positive B-cell NHL, including relapsed, refractory, and previously untreated cases. Doctors often use it with chemotherapy.
- Chronic Lymphocytic Leukemia (CLL): For CD20-positive chronic lymphocytic leukemia, your doctor may combine Ristova with chemotherapy agents such as fludarabine and cyclophosphamide.
Autoimmune Diseases
- Rheumatoid Arthritis (RA): If your rheumatoid arthritis does not respond to TNF antagonist therapy, your doctor may add Ristova injection with methotrexate to slow joint damage.
- Granulomatosis with Polyangiitis (GPA) & Microscopic Polyangiitis (MPA): Ristova injection is approved for these rare but serious blood vessel inflammations (vasculitides) in adults.
- Pemphigus Vulgaris: For moderate-to-severe pemphigus vulgaris, a blistering autoimmune skin disorder, Ristova injection offers a proven treatment option.

How Ristova Injection Is Administered
You never self-administer the Ristova injection at home. A trained healthcare professional dilutes the concentrate in saline or dextrose solution and gives it as a slow IV infusion in a clinical setting.
Before each infusion, your doctor prescribes pre-medication, usually an antihistamine and paracetamol, sometimes a corticosteroid, to reduce the risk of infusion reactions. The infusion starts at a slow rate (50 mg/hr) and increases gradually if you tolerate it, up to 400 mg/hr.
Your healthcare team monitors you closely during and after the infusion. This monitoring is essential for your safety.
Ristova Injection Side Effects
Like all medicines, Ristova injection can cause side effects. Knowing them helps you and your care team respond quickly.
Infusion Reactions
The most common side effects occur during or shortly after the infusion. These infusion reactions may include:
- Fever and chills
- Sore throat
- Drop in blood pressure (hypotension)
- Chest pain
- Irregular heartbeat
- Headache, nausea, or skin rash
- Hair Loss
Most infusion reactions happen during the first infusion and are manageable with pre-medication and a slower infusion rate. However, severe reactions, including serious drops in blood pressure or breathing difficulties, can occur and require immediate medical attention.
Serious Side Effects
Beyond infusion reactions, Ristova injection can also cause:
- Serious infections: Rituximab suppresses your immune system, increasing your risk of bacterial, viral, and fungal infections. Notify your doctor if you develop a fever, persistent cough, or other signs of infection.
- Hepatitis B reactivation: Ristova can reactivate dormant hepatitis B, sometimes with fatal consequences. Your doctor must screen you for hepatitis B before starting treatment.
- Progressive Multifocal Leukoencephalopathy (PML): A rare but serious brain infection linked to immune system suppression.
- Severe skin reactions: Including Stevens-Johnson syndrome. Contact your doctor immediately if you notice unusual skin blistering or peeling.
- Tumor lysis syndrome: Rapid breakdown of cancer cells can cause kidney and electrolyte problems, especially in patients with high tumor burden.
Always consult your doctor about any unusual symptoms or side effects. This list does not cover all possible reactions.
Precautions and Who Should Not Use Ristova Injection
Before starting treatment, your doctor reviews your full medical history. Please consult your doctor if any of the following apply:
- Allergy to rituximab or murine proteins: Known hypersensitivity is a contraindication.
- Active infections: Ristova should not start while you have an active, uncontrolled infection.
- Hepatitis B: All patients need hepatitis B screening before the first dose.
- Pregnancy or breastfeeding: Rituximab can harm the unborn baby. Use effective contraception during treatment and for at least 12 months after your last dose. Do not breastfeed during treatment or for 6 months after the last dose.
- Heart conditions: If you have a history of chest pain, irregular heartbeat, or other cardiac problems, your doctor will monitor you with extra care.
- Live vaccines: Do not receive any live vaccines during or after Ristova treatment without your doctor’s approval.
